SLED will send a Tattooist or Master Licence Renewal/Restoration Application form to your recorded email address about 60 days before the licence expiry date.
You must fill out the form and post it to SLED with your payment details. SLED must receive the form on or before the expiry date. If your licence expires, you will no longer be authorised to perform body art tattooing procedures.
If your licence expires you may apply to restore the licence using the Tattooist or Master Licence Renewal/Restoration Application form.
Applications to restore a licence must be received at SLED within three months of the licence expiry date.
If your licence was surrendered or cancelled, it cannot be restored.

Applications to restore a licence more than three months after the licence expiry date are only accepted in exceptional circumstances.
You must request approval to submit a late restoration application. Your request must be in writing and outline:
- why you didn’t apply to renew or restore your licence within three months of the expiry date, AND
- why your late restoration application should be accepted.
Requests must be addressed to the Manager Licensing Services via sled@police.nsw.gov.au. Decisions will be provided within five business days.
